TELEHEALTH
We can provide occupational therapy through telephone or video conferencing. Telehealth can be used in many situations including: concussion education and management, cognitive rehabilitation and strategies, ergonomic education, activation programming, wellness, coaching and goal setting, as well as self-management and education on sleep, memory, fatigue, pain, and headaches.
CASE MANAGEMENT
We conduct an assessment and make recommendations for equipment, support, and rehabilitation services, coordinating with other health care providers to maximize recovery. Case management is beneficial when catastrophic injuries or illnesses significantly impact return to prior function.
HOME & COMMUNITY REHABILITATION
We provide assessment, treatment, rehabilitation planning and activation to return to meaningful home and community activities. We set goals with clients and help in achieving them by gradually increasing day-to-day activities. This may include a home or gym exercise program.
HOSPITAL DISCHARGE COORDINATION
We coordinate a safe discharge from the hospital. We make sure the required home safety equipment, mobility equipment, and supports/services are in place prior to discharge. Once home, we can continue to work with clients to return to pre-injury/illness functioning.
RETURN TO WORK
PLANNING
We assess client's abilities and impairments and determine job demands required for work. Rehabilitation is provided to improve impairments and overcome work related barriers. We design graduated return to work plans to assist clients in progressing towards a successful return. A work site assessment may be completed to better understand work demands.
CONCUSSION & POST-CONCUSSION MANAGEMENT
We provide education and strategies to manage and overcome concussion and post-concussion symptoms such as fatigue, headaches, cognitive difficulties, visual difficulties, and sleep difficulties or changes in mood that impact activities of daily living. We make recommendations for accommodation at work/school and plan and guide a gradual resumption of activities.
COGNITIVE
REHABILITATION
We provide cognitive assessment and treatment. We look at client’s life demands and activities they need or want to do and design a rehabilitation program involving cognitive function recovery and compensation strategies to meet those demands and activities.
UPPER EXTREMITY REHABILITATION
We assess and treat upper extremity impairments related to stroke, brain injury, and other neurological disorders to regain function of hands, arms, shoulders, and trunk. Additionally, we can provide compensatory strategies to improve function. We can also assist with managing and improving shoulder pain.
VISION REHABILITATION
We provide screening and assessment of visual impairments due to aging or neurological injuries/conditions that impact day-to-day functions. Common impairments we treat include visual field deficits, double vision, and low vision. We may also teach compensatory strategies or suggest environmental changes.
POST COVID-19 REHABILITATION
We develop and implement graduated rehabilitation plans to help people overcome post Covid-19 difficulties including shortness of breath, strength, endurance, activity tolerance, mobility, and cognition difficulties. We can also provide strategies and education on pacing, energy conservation, sleep and cognition to better manage symptoms.
HOME SAFETY & ADAPTIVE EQUIPMENT
We assess home safety and recommend adaptive equipment to ensure a safe home, prevent re-injury or falls, help caregivers to provide safe care, and enable clients to maximize their independence. Recommendations may include removing falls hazards, increasing lighting, implementing bathing and toileting equipment, considering home modifications such as ramps, and educating caregivers on safe use of equipment and transfer techniques.
WHEELCHAIR & POWER MOBILITY ASSESSMENTS
We assess clients and their environment to determine wheelchair, power mobility (power wheelchair or scooter), and seating requirements to optimize functional mobility. These assessments may be required after an injury or illness that impacts ability to walk or when the previous wheelchair or seating system is no longer appropriate due to aging equipment or changing needs of clients.
VEHICLE MODIFICATIONS
We assess and determine vehicle modifications required for driving or being a passenger in a vehicle. We can provide recommendations for low tech driving vehicle modifications such as spinner knobs, hand controls, signal light extension, and left foot accelerator. We can also make recommendations on van modifications, lifts, ramps, 6-way seats, or tie downs for passengers and drivers.
